Stale facts in memory
You change your address, role, or project. The memory keeps the old fact. Without contradiction detection, you have a hallucination source.
Common pattern: user mentions something obliquely (e.g. "I was at the beach today" implies move to coast) without explicit memory update. Now memory holds two contradicting facts. Resolution depends on system: StudioMeyer Memory and Mem0 Pro auto-detect contradictions, others do not. Mitigations: bi-temporal queries (asOf), monthly nex_contradictions scans, aggressive confidence_decay on lifecycle facts (address, role, current project), watch-for-symptoms ("AI just referenced something wrong").
